Title:Vpliv senzorne integracije na kakovost dnevnega funkcioniranja bolnikov s fibromialgijo : doktorska disertacija študijskega programa tretje bolonjske stopnje Socialna gerontologija

Abstract:Sindrom fibromialgija (SFM) označuje kronično difuzno bolečino s povezovanjem patofiziološkega mehanizma z disfunkcijo centralnega in perifernega živčnega sistema. Za ugotavljanje potencialne vloge disfunkcije senzorne integracije pri bolnikih s SFM smo izvedli longitudinalno, randomizirano, eksperimentalno študijo z definiranjem razmerja senzorne modulacijske motnje (MSM) in izražanjem simptomatike SFM v perspektivi odkrivanja učinkovitejše obravnave bolnikov. Namensko izbrani vzorec 182 oseb, razdeljen na testno in kontrolno skupino 104 bolnikov s SFM ter primerjalno skupino 78 zdravih preiskovancev, smo izmerili s FIQ (Fibromyalgia Impact Qusetionnaire), AASP (Adolescent Adult Sensory Profile) in PSS (Perceived Stress Scale). Analizo podatkov smo izvedli z Mann-Whitney U in Kruskal-Wallis testom ter Wilcoxonovim preizkusom, povezanost ugotavljali s Pearsonovim hi-kvadratom z Likelihood Ratio, zanesljivost preverili s Cronbach alfa koeficientom, faktorsko analizo s Kaiser-Meyer-Olkin in Bartlettovim χ2 preizkusom, vpliv spremenljivk z linearno regresijo in diskriminantno analizo. Bolniki s FM so poročali o več simptomih, povezanih z MSM. Prisotnost in stopnja MSM pri bolnih je znatno višja, statistično pomembna (p < 0,001) v primerjavi z zdravimi osebami. Vpliv posameznih faktorjev MSM je na izražanje simptomatike SFM statistično značilen (p < 0,001), izraža višje zaznavanje stresa in nižjo psihofizično dnevno kakovost življenja bolnikov v primerjavi z zdravimi osebami. Učinek šestmesečne terapevtske intervencije izraža visok vpliv na redukcijo simptomatike SFM, specifično na bolečino, utrujenost, anksioznost in zaznavanje stresa. Razlike s kontrolno skupino so v vseh kategorijah statistično značilne (p < 0,001). Ugotovitve predstavljajo relevantne mejnike o izhodiščih potencialno novega večdimenzionalnega modela v multidisciplinarnem procesu zdravljenja bolnikov s SFM, z namenom izboljšanja s psihofizičnim zdravjem povezane kakovosti dnevnega funkcioniranja.

Abstract:Fibromyalgia syndrome (FMS) characterizes chronic diffuse pain by associating a pathophysiological mechanism with central and peripheral nervous system dysfunction. To determine the potential role of sensory integration dysfunction in patients with FMS, we conducted a longitudinal, randomized, experimental study by defining the relationship of sensory modulation disorder (SMD) and the expression of FM symptoms in the perspective of discovering more effective treatment of patients. A sample of 182 people, divided into a test and control group of 104 patients with FMS and a comparison group of 78 healthy subjects, was measured with the FIQ (Fibromyalgia Impact Questionnaire), AASP (Adolescent Adult Sensory Profile) and PSS (Perceived Stress Scale). Data analysis was performed with the Mann-Whitney U and Kruskal-Wallis test and the Wilcoxon test, the relationship was determined with the Pearson chi-square with the Likelihood Ratio, the reliability was checked with the Cronbach alpha coefficient, factor analysis with the Kaiser-Meyer-Olkin and Bartlett's χ2 test, the influence variables with linear regression and discriminant analysis. Patients with FMS reported many symptoms associated with SMD. The presence and impact of SMD in patients with FMS is significantly higher, statistically significant (p <0.001), compared to healthy subjects. The influence of individual SMD factors on the expression of FMS symptoms is statistically significant (p <0.001), it shows a higher perception of stress and a lower psychophysical daily quality of life of patients, compared to healthy people. The effect of the six-month therapeutic intervention shows a high impact on the reduction of FM symptoms, specifically on pain, fatigue, anxiety and stress perception. Differences with the control group are statistically significant in all categories (p <0.001). The findings represent relevant milestones on the starting points of a potentially new multidimensional model in the multidisciplinary process of treating patients with FMS, with the aim of improving the quality of daily functioning related to psychophysical health.
