Title:Fizioterapevtska obravnava pacienta s kalcinirajočim tendinitisom rotatorne manšete

Abstract:Ramenski sklep je najbolj gibljiv kroglični sklep v telesu. Zaradi svoje anatomije je pogosto podvržen različnim patološkim spremembam. Kalcinirajoči tendinitis (KT) rotatorne manšete (RM) je ena najpogostejših degenerativnih patologij ramenskega sklepa, ki nastane zaradi nalaganja kristalov kalcijevega fosfata v tetive mišic RM, najpogosteje v tetivo mišice supraspinatus. To povzroča bolečino, omejeno gibljivost in zmanjšano funkcionalnost sklepa, kar pomembno vpliva na kakovost življenja bolnika. Pojavlja se predvsem pri odraslih med tridesetim in šestdesetim letom starosti, pogosteje pri ženskah. KT običajno poteka v več fazah, zdravljenje pa je lahko konzervativno ali operativno. V praksi se pogosto najprej uporabljajo konzervativni pristopi, med katere sodijo počitek, analgetiki, nesteroidna protivnetna zdravila, terapevtske vaje in fizioterapevtska obravnava. V zadnjih letih se vedno pogosteje uporablja tudi terapija z udarnimi valovi (ESWT), ki omogoča razgradnjo kalcinatov in učinkovito zmanjšuje bolečino. Namen diplomske naloge je bil s pregledom literature raziskati fizioterapevtske pristope k zdravljenju KT in ovrednotiti učinkovitost terapije z ESWT v primerjavi z drugimi metodami. V analizo je bilo vključenih osem kakovostnih kliničnih študij, objavljenih med letoma 2000 in 2025, iz podatkovnih baz PubMed, ScienceDirect. Vključene raziskave so bile randomizirane, nadzorovane ali prospektivne narave, večina tudi dvojno slepo zasnovanih. Analiza rezultatov je pokazala, da je ESWT učinkovita fizioterapevtska metoda za zdravljenje KT RM. Ugotovili smo, da je visokoenergijska oblika ESWT najbolj učinkovita, saj je hitreje statistično značilno zmanjšala bolečino, izboljšala funkcionalnost in pospešila razgradnjo kalcinacij v primerjavi z nizkoenergijsko obliko ali placebo terapijo. Rezultati sistematičnega pregleda podpirajo hipotezo, da je ESWT najbolj učinkovita fizioterapevtska metoda za zdravljenje KT. Diplomsko delo poudarja pomen celostne in individualno prilagojene fizioterapevtske obravnave, ki vključuje tako pasivne kot aktivne metode in upošteva specifične potrebe posameznega pacienta. Ugotovitve bodo v pomoč pri nadaljnjem kliničnem delu in oblikovanju smernic za učinkovitejšo rehabilitacijo bolnikov s KT RM.

Title:Physiotherapeutic treatment of patients with rotator cuff calcific tendinopathy
Abstract:The shoulder joint is the most mobile ball-and-socket joint in the human body and, due to its anatomical structure, is frequently affected by various pathological changes. Calcific tendinitis (CT) of the rotator cuff (RC) is one of the most common degenerative shoulder pathologies, caused by the deposition of calcium phosphate crystals in the RC tendons, most often in the supraspinatus tendon. This leads to pain, reduced range of motion, and diminished joint functionality, significantly impacting the patient’s quality of life. The condition most commonly affects adults between the ages of thirty and sixty and is more prevalent in women. CT usually progresses through several phases, and treatment can be either conservative or surgical. In clinical practice, conservative approaches are typically applied first, including rest, analgesics, nonsteroidal anti-inflammatory drugs, therapeutic exercises, and physiotherapy. In recent years, extracorporeal shockwave therapy (ESWT) has become increasingly popular, as it promotes calcification resorption and effectively reduces pain. The aim of this thesis was to review the literature and investigate physiotherapeutic approaches to the treatment of CT, with a specific focus on evaluating the effectiveness of ESWT compared to other methods. The analysis included eight high-quality clinical studies published between 2000 and 2025, retrieved from PubMed and ScienceDirect databases. The included studies were randomized, controlled, or prospective in design, with most also being double-blinded. The analysis showed that ESWT is an effective physiotherapeutic method for treating RC CT. High-energy ESWT was found to be the most effective, as it significantly reduced pain, improved functionality, and accelerated calcification resorption compared to low-energy or placebo treatments. The findings of this systematic review support the hypothesis that ESWT is the most effective physiotherapeutic method for treating calcific tendinitis. This thesis highlights the importance of a comprehensive and individualized physiotherapeutic approach, incorporating both passive and active methods tailored to the specific needs of each patient. The results can contribute to further clinical practice and to the development of more effective rehabilitation guidelines for patients with RC CT.
