Title:Primerjava uspešnosti matičnih celic iz različnih virov za zdravljenje osteoartroze kolenskega sklepa

Abstract:Teoretična izhodišča: Osteoartroza (OA) je degenerativna bolezen sinovialnih sklepov, ki povzroča propadanje hrustanca, bolečino in zmanjšano gibljivost. Zdravljenje z matičnimi celicami (MC) iz kostnega mozga ali maščobnega tkiva predstavlja minimalno invaziven pristop za zmanjšanje vnetja, regeneracijo hrustanca in izboljšanje funkcije sklepa. Namen raziskave je bil primerjati učinkovitost obeh metod pri zdravljenju OA kolena. Metoda: V retrospektivno raziskavo so bili vključeni bolniki z OA kolena stopnje II–III po klasifikaciji Kellgren-Lawrence, zdravljeni z MC iz kostnega mozga ali maščobnega tkiva (izbira glede na želje bolnika in stroške). Zbrani so bili podatki o demografiji, bolečini (vizualna analogna lestvica – VAL), funkciji kolena (KOOS) ter stopnji OA (RTG in napredne slikovne preiskave). Klinični izid je bil ocenjen pred zdravljenjem, 3 in 12 mesecev po njem. Rezultati: Stopnja OA ni bila povezana s predoperativno bolečino ali funkcijo kolena. Obe metodi sta po 3 mesecih povzročili statistično značilno zmanjšanje bolečine (VAL) in izboljšanje funkcije (KOOS), učinek pa se je ohranil po enem letu. Čeprav je bilo zmanjšanje bolečine absolutno večje pri skupini z MC iz maščobnega tkiva, razlike med skupinama glede funkcionalnih izidov po 12 mesecih niso bile statistično pomembne. Začetna resnost degenerativnih sprememb ni vplivala na zaznano zmanjšanje bolečine. Zaključek: Oba vira MC sta učinkovita pri zdravljenju OA kolena, saj zmanjšujeta bolečino in izboljšujeta funkcijo brez pomembnih razlik po enem letu. Omejitve raziskave vključujejo nerandomiziran dizajn, odsotnost kontrolne skupine in enocentrično izvedbo. Potrebne so prihodnje prospektivne, randomizirane študije z daljšim spremljanjem.

Title:Comparison of the etfectiveness of different stem cel/ sources for the treatment of osteoarthritis of the knee joint
Abstract:Theoretical Background: Osteoarthritis (OA) is a degenerative disease of synovial joints that causes cartilage degradation, pain, and reduced mobility. Treatment with mesenchymal stem cells (MSC) derived from bone marrow or adipose tissue is a minimally invasive approach aimed at reducing inflammation, regenerating cartilage, and improving joint function. The aim of this study was to compare the clinical effectiveness of both methods in knee OA treatment. Methods: This retrospective study included patients with knee OA grade II–III according to the Kellgren–Lawrence classification, treated either with bone marrow–derived MSCs or adipose tissue–derived MSCs (treatment choice based on patient preference and cost). Data collected included demographics, pain (visual analogue scale – VAS), knee function (Knee Injury and Osteoarthritis Outcome Score – KOOS), and OA severity (X-ray and advanced imaging). Assessments were performed before treatment, and at 3- and 12-months post-treatment. Results: OA grade was not associated with preoperative pain or knee function. Both methods led to statistically significant pain reduction (VAS) and functional improvement (KOOS) at 3 months, with effects sustained at 12 months. Although absolute pain reduction was slightly greater in the adipose-derived MSC group, no statistically significant differences in functional outcomes between the groups were observed at 12 months. Baseline OA severity did not influence perceived pain reduction. Conclusion: Both bone marrow– and adipose-derived MSC therapies effectively reduced pain and improved function in knee OA, with comparable outcomes at 12 months. Study limitations include non-randomized design, lack of a control group, and single-center setting. Future multicenter randomized trials with longer follow-up are recommended.
