Title:Učinkovitost manualne terapije pri zdravljenju osteoartroze kolka – pregled literature : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Osteoartroza (OA) kolka je ena najpogostejših oblik degenerativne sklepne bolezni in pomemben vzrok za bolečino, zmanjšano gibljivost ter funkcionalne omejitve pri odrasli populaciji. Manualna terapija (MT) predstavlja enega izmed temeljnih pristopov fizioterapije in se pogosto uporablja pri zdravljenju nevro-mišično-skeletnih motenj. Kljub široki uporabi pa učinkovitost MT ni najbolj znana pri OA kolka. Namen tega diplomskega dela je s sistematičnim pregledom literature ovrednotiti učinkovitost različnih tehnik MT pri zdravljenju OA kolka. Pri sistematičnem pregledu literature smo pregledali, analizirali in ovrednotili različne metode in tehnike MT ter ocenili njihov vpliv na zdravljenje OA kolka. Pri pregledu smo uporabili le randomizirane kontrolirane študije (RCT) in kvazi-RCT. Ugotovili smo, da imajo različne tehnike MT pozitivne učinke na bolečino, obseg gibljivosti (ROM), funkcionalno mobilnost in funkcionalne indekse. Rezultati študij večinoma potrjujejo le kratkoročne pozitivne učinke do nekaj tednov po intervenciji (2–4 tedne). Dolgotrajnejši učinek (3 mesece po intervenciji) je bil potrjen izključno pri mobilizaciji z gibanjem (MWM) v kombinaciji s samomobilizacijo. V prihodnosti bi za vrednotenje vpliva MT potrebovali več študij, ki bi preverjale več različnih intervencijskih protokolov ter spremljale učinek tudi več kot 6 mesecev po zaključku intervencij.

Abstract:Hip osteoarthritis (OA) is one of the most common forms of degenerative joint diseases and a significant cause of pain, reduced mobility, and functional limitations in the adult population. Manual therapy (MT) is one of the fundamental approaches in physiotherapy and is frequently used in the treatment of neuromusculoskeletal disorders. However, despite its widespread use, the effectiveness of MT in the management of hip OA remains relatively unclear. The aim of this thesis is to systematically review the literature to evaluate the effectiveness of different manual therapy techniques in the treatment of hip OA. In this systematic literature review, we analyzed and evaluated various MT methods and techniques and assessed their impact on the management of hip OA. Only randomized controlled trials (RCTs) and quasi-RCTs were included in the review. Our findings indicate that various MT techniques have positive effects on pain, range of motion (ROM), functional mobility, and functional indices. Most studies confirm only short-term effects, lasting up to a few weeks after the intervention (2–4 weeks). Mobilization with movement (MWM) in combination with self-mobilization was the only approach that demonstrated sustained positive effects for 3 months post-intervention. Future research should focus on measuring different intervention protocols, long-term outcomes, with follow-up periods longer than 6 months to better evaluate the sustained effects of MT.
