Title:Vpliv fizioterapevtske obravnave na celjenje perinealne rane po vaginalnem porodu z epiziotomijo : pregled literature

Abstract:Teoretično izhodišče: Disfunkcija medeničnega dna je vse pogostejši zdravstveni problem žensk, ki je povezan z dejavniki nosečnosti in poroda, med katerimi pomembno vlogo igra epiziotomija. Epiziotomija kot kirurški poseg lahko povzroči številne fizične in psihične zaplete, ki negativno vplivajo na kakovost življenja žensk po porodu. Namen diplomskega dela je bil raziskati vrste in učinke fizioterapevtskih metod in tehnik na celjenje perinealne rane po vaginalnem porodu z epiziotomijo. Delo se osredotoča na ugotavljanje njihove učinkovitosti, pogostosti uporabe ter primerjavo različnih pristopov glede na njihove prednosti in morebitne omejitve. Metode dela: Potek zbiranja in obdelave podatkov je obsegal sistematično iskanje, izbor ter analizo ustrezne znanstvene in strokovne literature, dostopne prek podatkovnih baz, kot so PubMed, Google Scholar, ScienceDirect, PEDro in ResearchGate. Za ponazoritev izbora podatkovnih virov in poteka odločanja o vključitvi literature smo uporabili PRIZMA diagram. Rezultati: Različne fizioterapevtske metode dokazano izboljšujejo celjenje perinealne rane po epiziotomiji, pri čemer sta med najučinkovitejšimi trening mišic medeničnega dna in terapevtski ultrazvok. Trening mišic medeničnega dna omogoča varno in samostojno izvajanje, medtem ko je pri ultrazvoku potreben strokovni nadzor. Druge metode, kot so infrardeča svetloba, krioterapija in LLLT, dopolnjujejo zdravljenje z različnimi učinki na bolečino in prekrvavitev. Razprava: Pregled literature je pokazal, da vse uporabljene metode prispevajo k hitrejšemu celjenju in zmanjševanju bolečin. Kombinacija treninga mišic medeničnega dna in ultrazvoka je najučinkovitejša, ultrazvok je prevladujoč glede biostimulacije v primerjavi z laserjem, krioterapija pa nudi kratkotrajen protibolečinski učinek.

Abstract:Theoretical Background: Pelvic floor dysfunction is an increasingly common health issue among women that is associated with pregnancy and childbirth factors, among which episiotomy plays an important role. Episiotomy as a surgical procedure can cause various physical and psychological complications that affect women’s quality of life postpartum. The purpose of this thesis was to investigate the types and effects of physiotherapeutic methods and techniques on the healing of perineal wounds after vaginal delivery with episiotomy. The focus was on determining their effectiveness and frequency of use, and on comparing different approaches based on their advantages and potential limitations. Methods: Data collection and processing involved systematic search, selection, and analysis of relevant scientific and professional literature available through databases such as PubMed, Google Scholar, ScienceDirect, PEDro, and ResearchGate. The PRISMA diagram was used to illustrate the selection process of data sources and decision-making regarding the inclusion of literature. Results: Various physiotherapeutic methods have been proven to improve the healing of perineal wounds after episiotomy, with pelvic floor muscle training and therapeutic ultrasound being among the most effective. Pelvic floor muscle training allows safe and independent practice, whereas ultrasound requires professional supervision. Other methods, such as infrared light, cryotherapy, and low-level laser therapy (LLLT), complement treatment with different effects on pain and blood circulation. Discussion: The literature review showed that all applied methods contribute to faster healing and pain reduction. The combination of pelvic floor muscle training and ultrasound is the most effective, with ultrasound being superior in terms of biostimulation compared to laser therapy, while cryotherapy provides a short-term analgesic effect.
