Title:Učinki fizioterapevtske obravnave pri utesnitvenem sindromu rotatorne manšete: pregled literature : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Utesnitveni sindrom rotatorne manšete je pogost vzrok bolečine in omejene gibljivosti ramenskega sklepa, ki lahko močno vpliva na kakovost življenja. Za učinkovito obravnavo tega stanja je ključna fizioterapevtska rehabilitacija, saj lahko prepreči razvoj kroničnih težav in potrebo po kirurškem zdravljenju. V diplomskem delu je bil izveden sistematični pregled literature o učinkih različnih konzervativnih fizioterapevtskih metod pri zdravljenju tega sindroma. Pregled se je osredotočil na kratkoročne in dolgoročne učinke teh pristopov na zmanjšanje bolečine ter izboljšanje funkcionalnosti in na prepoznavanje najučinkovitejših metod zdravljenja. Analiza osmih izbranih raziskav je pokazala, da vse preučevane metode, ki so bile terapevtske vaje, manualna terapija in fizikalne modalitete, prispevajo k zmanjšanju bolečine ter izboljšanju funkcije rame. Največje in najbolj trajne izboljšave zagotavlja redno izvajanje individualno prilagojenega programa terapevtskih vaj. Celosten, individualiziran pristop, ki združuje aktiven vadbeni program s ciljno izbranimi dodatnimi metodami glede na potrebe bolnika, se je izkazal za najuspešnejšega. Takšen pristop optimalno zmanjša bolečino, povrne funkcionalno sposobnost rame in zmanjša tveganje za ponovitev težav. Ugotovitve pregleda literature poudarjajo, da je strokovno voden program terapevtskih vaj temelj uspešne rehabilitacije utesnitvenega sindroma, dodatne fizikalne in manualne tehnike pa imajo podporno vlogo. Ti izsledki lahko usmerijo klinično fizioterapevtsko prakso k razvoju standardiziranih rehabilitacijskih protokolov, ki temeljijo na aktivnem pristopu.

Abstract:Rotator cuff impingement syndrome is a common cause of shoulder pain and limited mobility, significantly affecting quality of life. Timely and approprate physiotherapy rehabilitation is crucial to prevent chronic issues and the need for surgery. A systematic literature review was conducted on the effects of various conservative physiotherapy interventions for treating shoulder impingement. The review focused on the short-term and long-term effects of these interventions on pain and function, and aimed to identify the most effective approaches. Analysis of eight studeis showd that all examined interventions, such as therapeutic exercises, manual therapy and physical modalities alleviate pain and improve shoulder function. However, the most significant and lasting improvements were achieved throug a regular, individualized therapeutic exercise program. An individualized approach combining an active exercise program with targeted selected therapies as needed proved most effective. Such an approach optimally reduces pain, restores function, and decreases the risk of recurrence. The findings highlight that a guided exercise program is the cornerstone of successful rehabilitation for impingement syndrome, with additional physical and manual therapies playing a supportive role. These results can inform clinical pracitce in developing standardized rehabilitation protocols that emphasize an active approach.
