Title:Učinkovitost hipoterapije na ravnotežje in funkcijsko premičnost pri pacientih z multiplo sklerozo – pregled literature : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Multipla skleroza (MS) je kronična avtoimunska bolezen osrednjega živčnega sistema, za katero je značilna demielinizacija bele snovi v možganih in hrbtenjači. Med zgodnje in najbolj omejujoče simptome sodijo motnje ravnotežja ter zmanjšana funkcijska premičnost, ki lahko pomembno vplivata na samostojnost in opravljanje vsakodnevnih dejavnostih. Hipoterapija predstavlja dopolnilni pristop, ki se je izkazal kot učinkovit pri obvladovanju teh težav. Namen diplomske naloge je bil raziskati učinkovitost hipoterapije kot dopolnilne metode pri izboljšanju ravnotežja in funkcijske premičnosti pri osebah z multiplo sklerozo. Literaturo smo v bazah PubMed, PEDro in CINAHL iskali z vnaprej določenimi ključnimi besedami in Boolovimi operaterji, kar je omogočilo ciljno iskanje ter zajem različnih poimenovanj hipoterapije. V pregled literature smo vključili šest raziskav, od tega štiri randomizirane nadzorovane raziskave, eno klinično raziskavo in eno študijo kliničnih primerov. Randomizirane raziskave (RCT) so bile po PEDro lestvici ocenjene med 5 in 8/10, medtem ko sta bili preostali dve raziskavi ocenjeni z 9 in 10/12 po SURE kriterijih. V vseh raziskavah so izvajali hipoterapijo na živem konju. Vključenih je bilo 161 oseb z MS, starosti 32–60 let, terapevtski programi pa so trajali od 3 do 16 tednov. Pregled literature dosledno potrjuje, da hipoterapija pri osebah z multiplo sklerozo predvsem izboljšuje ravnotežje in funkcijsko premičnost, kar se kaže v statistično pomembnih izboljšavah predvsem v eksperimentalnih skupinah.

Abstract:Multiple sclerosis is a chronic autoimmune disease of the central nervous system, characterized by demyelination of the white matter in the brain and spinal cord. Early and most disabling symptoms include balance impairments and reduced functional mobility, which can significantly affect independence and performance in daily activities. Hippotherapy represents a complementary approach that has proven effective in managing these difficulties. The aim of this thesis was to investigate the effectiveness of hippotherapy as a supplementary intervention for improving balance and functional mobility in individuals with MS. Literature was searched in the PubMed, PEDro, and CINAHL databases using predefined keywords and Boolean operators, allowing targeted searches and the inclusion of various terms for hippotherapy. Six studies were included in the review: four randomized controlled trials (RCTs), one clinical study, and one case study. The RCTs were rated 5–8/10 on the PEDro scale, while the other two studies scored 9 and 10/12 according to SURE criteria. All studies implemented hippotherapy on a live horse. A total of 161 participants with MS, aged 32–60 years, were included, and therapeutic programs lasted between 3 and 16 weeks. The review consistently shows that hippotherapy primarily improves balance and functional mobility in individuals with MS, with statistically significant improvements observed mainly in the experimental groups.
