Title:Fizioterapevtska obravnava ploskega stopala

Abstract:Tema zaključnega dela je fizioterapevtska obravnava ploskega stopala, ki se pogosto pojavlja tako v otroštvu kot v odrasli dobi, a ostaja premalo prepoznana. Pomembno vlogo prepoznavanja patologije ploskega stopala predstavljajo poznavanje anatomije stopala, dobro zastavljen in izpeljan klinični pregled ter individualno sestavljen program terapije. Posameznikom priporočamo, da najprej posegajo po konservativnih metodah, saj je tak pristop v veliki večini uspešen. Vseeno pa nimamo zanesljivih dokazov, kateri fizioterapevtski program naj bi bil pri tovrstni patologiji splošno priporočljiv za ukrepanje. Glavno težavo predstavlja pomanjkanje enotnih smernic in ustrezno dokazanih fizioterapevtskih metod obvladovanja tovrstnih deformacij. Namen naloge je bil pregledati znanstveno literaturo in ugotoviti učinkovitost fizioterapevtskih metod in tehnik pri posameznikih s ploskim stopalom. Cilj je bil pripraviti priporočila za praktično uporabo v klinični fizioterapevtski obravnavi. Raziskovalno vprašanje se je glasilo, ali obstajajo zanesljivi dokazi o učinkovitosti fizioterapevtske obravnave pri posameznikih s ploskim stopalom. Na podlagi vključitvenih kriterijev je bilo v sistematični pregled vključenih dvanajst študij. Večina študij je potrdila pozitiven vpliv vaj za krepitev intrinzičnih mišic stopala, predvsem vaje za kratko stopalo, ki so izboljšale višino navikularne kosti, kot vzdolžnega loka in splošno funkcijo stopala. Dodatno so se kot koristne izkazale individualno prilagojene ortoze in nevromišična stimulacija, čeprav le v kombinirani obravnavi. Hipotezo, da obstajajo zanesljivi dokazi o učinkovitosti fizioterapevtske obravnave pri obravnavi posameznikov s ploskim stopalom, smo morali zavrniti zaradi metodoloških omejitev in majhnega števila kakovostnih študij. Kljub temu so rezultati spodbudni in nakazujejo na potrebo po nadaljnjem raziskovanju. Pridobitev za stroko predstavlja oblikovan teoretični okvir fizioterapevtske obravnave, ki vključuje dokazano učinkovite metode ter priporočila za prakso.

Title:Physiotherapy treatment of pes planus
Abstract:The topic of this thesis is the physiotherapy treatment of flat foot, a common but often underrated condition in both children and adults. An important aspect in recognizing flatfoot patology lies in a solid understanding of foot anatomy, a well-structured and through clinical examination and an individually tailored therapy program. It is recommended that individuals initially opt for conservative treatment methods, as this approach proves effective in the majority of cases. However, there is still insufficient reliable evidence to support a universally recommended physiotherapy protocol for managing this condition. The main issue addressed is the lack of consistent guidelines and evidence-based phyisiotherapeutic methods to manage this deformity. The purpose of this thesis was to review relevant scientific literature and assess the effectivness of physiotherapy intevention for individuals with flat foot. The goal was to develop practical recommendations for clinical physiotherapy practice. The research question focused on whether reliable evidence exists fort he effectivness of physiotherapy treatment in flat foot management. Based on inclusion criteria, 12 studies were analyzed in the systematic review. Most studies demostrated positive effects of strengthing exercises for intinsic foot muscles, especially short-foot exercises which led to improvements in navicular drop height, longitudinal arch drop and general foot functiont. Additionally, customized orthoses and neuromuscular stimulation showed supportive benefits combined. The hypothesis that reliable evidence exists had to be rejected gue to limited number and methodological variability of high-quality studies. Nevertheless, the findings are promising and underline the need for further research. For the profession, this thesis provides a theoretical framework for physiotherapy intervention and offers evidence-based guidelines for treating patients with flat foot in clinigal settings.
