Title:Učinkovitost manualne terapije pri zdravljenju rizartroze : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Rizartroza (RA) je degenerativna sprememba trapeziometakarpalnega (TMC) sklepa palca, ki povzroča bolečino, zmanjšano gibljivost in moteno funkcijo roke. Ti simptomi pomembno vplivajo na vsakodnevno življenje bolnikov, kar poudarja potrebo po učinkovitih terapevtskih pristopih. Manualna terapija (MT) predstavlja enega izmed konservativnih pristopov fizioterapije, ki se pogosto uporablja pri obravnavi degenerativnih sprememb. Namen diplomskega dela je bil predstaviti anatomske in patofiziološke značilnosti RA, pregledati konservativne in operativne možnosti zdravljenja ter posebej oceniti učinkovitost MT na podlagi dostopnih raziskav. V analizo je bilo vključenih pet raziskav, od tega štiri randomizirane kontrolirane študije in ena pilotna. Preiskovanci so bili večinoma ženskega spola, starejši od 65 let, z napredovalo obliko bolezni (III.–IV. stopnja po Eaton-Littler-Burtonu). MT je obsegala mobilizacijo TMC sklepa in nevrodinamske tehnike, časovni razpon intervencij pa je bil od dva do štiri tedne. Raziskave so spremljale učinke MT na bolečino, prag bolečine ob pritisku (PPT), moč pincetnega in tripod prijema ter moč stiska roke. Rezultati so pokazali kratkoročno zmanjšanje bolečine in povečanje PPT, učinki na moč pincetnega prijema, tripod prijema in roke pa so bili manj enotni. Na podlagi ugotovitev lahko sklepamo, da je MT varna in učinkovita komplementarna metoda pri obravnavi RA, saj kratkoročno zmanjšuje bolečino, vendar so za določitev optimalnih protokolov potrebne nadaljnje raziskave z večjimi in bolj raznolikimi vzorci ter dolgoročnejšim spremljanjem učinkov.

Abstract:Rhizarthrosis (RA) is a degenerative change of the trapeziometacarpal (TMC) joint of the thumb, causing pain, reduced mobility, and impaired hand function. These symptoms significantly affect patients’ everyday lives, highlighting the need for effective therapeutic approaches. Manual therapy (MT) represents one of the conservative physiotherapy approaches frequently used in the treatment of degenerative conditions. This thesis aimed to present the anatomical and pathophysiological characteristics of RA, review conservative and surgical treatment options, and specifically evaluate the effectiveness of MT based on available research. The analysis included five studies, four of which were randomized controlled trials and one pilot study. The participants were predominantly women over the age of 65 with advanced disease (stage III–IV according to the Eaton-Littler-Burton classification). MT interventions included mobilization of the TMC joint and neurodynamic techniques, with treatment durations ranging from 2 to 4 weeks. The studies evaluated the effects of MT on pain, pressure pain threshold (PPT), pinch strength, tripod strenght and grip strength. Results showed short-term reductions in pain and increases in PPT, while effects on pinch, tripod and grip strength were less consistent. Based on these findings, MT can be considered a safe and effective complementary method for managing RA, as it provides short-term pain relief; however, further research with larger and more diverse samples and long-term follow-up is needed to determine optimal treatment protocols.
