Abstract:Uvod: Cerebralna paraliza je eden najpogostejših vzrokov za gibalno oviranost pri otrocih. Pri spastični obliki bolezni, togost mišic vpliva na gibanje, držo in funkcionalne sposobnosti oseb s cerebralno paralizo. Razvoj klinično preverjenih terapevtskih pristopov pri obravnavi spastičnosti mišic je izredno pomemben z vidika izboljšanja kakovosti življenja oseb s cerebralno paralizo. Namen diplomskega dela je bil raziskati učinek udarnih valov na spastičnost mišic pri pacientih s cerebralno paralizo ter primerjati parametre, uporabljene v različnih raziskavah . Metode: V pregled je bilo vključenih osem empiričnih študij, ki so preučevale učinke terapije z udarnimi valovi na spastičnost mišic pacientov s cerebralno paralizo, narejen je bil pregled literature po smernicah PRISMA. Uporabljene so bile deskriptivna metoda, metoda komparacije, kompilacije ter analize dokumentov. Rezultati: Pregled literature je pokazal, da je terapija z udarnimi valovi učinkovita, varna in neinvazivna metoda za zmanjševanje spastičnosti mišic pri pacientih s cerebralno paralizo. Terapija z udarnimi valovi lahko učinkovito zmanjša mišični tonus, poveča pasiven obseg gibanja in izboljša funkcionalno gibljivost. Razprava: Tako radialna kot tudi fokusna oblika terapije z udarnimi valovi sta se izkazali kot učinkoviti za zmanjševanje spastičnosti pri pacientih s cerebralno paralizo, pri čemer je optimalno, da se parametri določijo v skladu z individualnimi potrebami pacienta in izkušnjami fizioterapevta. Ključne izzive predstavlja standardizacija protokolov, heterogenost vzorcev in dolgoročno spremljanje pacientov z namenom ugotavljanja trajanja učinkov terapije, kar odpira prostor za nadaljnje raziskovanje.

Abstract:Introduction: Cerebral palsy is one of the most common causes of motor disability in children. In the spastic form, muscle rigidity affects movement, posture and functional abilities. The development of clinically validated therapeutic approaches is of paramount importance in terms of improving the quality of life of people with cerebral palsy. The aim of this thesis was to investigate the effect of shock waves on muscle spasticity in cerebral palsy patients and to compare the parameters used in different studies. Methods: Eight empirical studies investigating the effects of shock wave therapy on spasticity in cerebral palsy patients were included in the review, and a systematic literature review was conducted according to PRISMA guidelines. Descriptive, comparative, compilation and document analysis methods were used. Results: The literature review revealed that shock wave therapy is an effective, safe and non-invasive method for reducing muscle spasticity in cerebral palsy. It can result in a reduction of muscle tone, improvement of passive range of movement and improvement of functional mobility. Discussion: Both radial and focused forms of therapy have been shown to be effective in reducing spasticity in patients with cerebral palsy, with the optimal parameters being set according to the individual needs of the patient and the experience of the physiotherapist. Standardisation of protocols, heterogeneity of samples and long-term follow-up of patients to determine the duration of the effects of therapy are key challenges, opening up opportunities for further research.
