Title:Celostna obravnava pacienta z motnjami srčnega ritma

Abstract:Teoretična izhodišča: Normalen srčni utrip odraslega človeku v mirovanju znaša 60-100 utripov na minuto in ima elektrofiziološke lastnosti normalnega EKG-ja. Vsako odstopanja od tega opredelimo kot motnje srčnega ritma ali aritmije. Motnje srčnega ritma oziroma aritmije predstavljajo skupino bolezenskih stanj, pri katerih prihaja do nepravilnega nastajanja ali prevajanja električnih impulzov v srcu. Posledično je prisoten prehiter, prepočasen ali nereden srčni utrip. Aritmije so lahko povezane z drugimi bolezenskimi stanji ali pa se pojavijo pri drugače zdravem posamezniku. Namen diplomske naloge je bil predstaviti najpogostejše motnje srčnega ritma, opisati diagnostično-terapevtske preiskave ter opredeliti vlogo medicinske sestre pri potrditvi diagnoze in uspešnem zdravljenju, namen raziskave pa raziskati celostno obravnavo pacienta z motnjo srčnega ritma. Metode: Raziskava je temeljila na kvalitativnem načinu raziskovanja in deskriptivni metodi dela. Za potrebe empiričnega dela so bili zbrani in analizirani primarni in sekundarni viri. Primarni podatki za analizo so bili zbrani s tehniko intervjuvanja. V raziskavo je bilo vključenih šest pacientov z motnjami srčnega ritma. Sekundarni podatki so bili pridobljeni s pomočjo pregleda domače in tuje strokovne literature ter spletnih baz. Rezultati: Intervjuvancem bolezen predstavlja vsakodnevno oviro pri svojih opravilih in življenjskih aktivnostih. Do ugotovitve diagnoze so prišli preko opravljenih preiskav. Celostno obravnavo dojemajo tako, da so obravnavni v celoti skupaj z njihovim načinom življenja. Intervjuvanci se srečujejo s poslabšanjem bolezni in tveganji za dodatne življenjsko ogrožajoče zaplete. Intervjuvancem se preventivni pregledi in izobraževalni programi zdijo koristni. Izobraževalnih programov se udeležuje le ena intervjuvanka. Referenčno ambulanto redno obiskujejo in so z obravnavo zadovoljni. Razprava: Ugotovljeno je bilo, da prisotnost motnje srčnega ritma pacientom predstavlja stalne prilagoditve na vsakodnevne aktivnosti. Obravnava do potrditve bolezni je potekala z različnimi diagnostično terapevtskimi-preiskavami, ki so bile podobne pri vseh intervjuvancih. Preiskave se med seboj povezujejo in po potrebi so večkrat izvajane za pridobitev novih izvidov. Pacienti celostno obravnavo njihove bolezni dojemajo kot zajemanje celote pacienta, skupaj z boleznijo, vključno z njihovimi navadami, fizičnim in psihičnim stanjem, vsakodnevnimi opravili in življenjskim slogom. Zapleti, s katerimi se pacienti vsakodnevno srečujejo, so palpitacije, omotica, slabost, bolečine v prsih, razbijanje srca, sinkopa. Vse to lahko pripelje do nenadne srčne smrti ali ishemične možganske smrti. Pacientom se preventivni pregledi zdijo pomembni, vendar se premalo udeležujejo dodatnih izobraževanj. Referenčne ambulante redno spremljajo in nadzorujejo zdravstveno stanje pacientov, vendar nimajo posebej obravnavane skupine za motnje srčne ritma

Title:Comprehensive Treatment of Patients with Heart Rhythm Disorders
Abstract:Baseline: The normal resting heart rate of an adult is 60-100 beats per minute and has the electrophysiological properties of a normal ECG. Any deviation from this is defined as a heart rhythm disorder or arrhythmia. Heart rhythm disorders or arrhythmias represent a group of disease states in which there is an irregular generation or conduction of electrical impulses in the heart. As a result, there is a heart rate that is too fast, too slow or irregular. Arrhythmias can be associated with other disease states or occur in an otherwise healthy individual. The purpose of the diploma thesis was to present the most common heart rhythm disorders, describe diagnostic and therapeutic examinations and define the role of the nurse in confirming the diagnosis and successful treatment, and the purpose of the research was to investigate the comprehensive treatment of a patient with a heart rhythm disorder. Methods: The research was based on a qualitative research method and a descriptive work method. For the needs of the empirical work, primary and secondary sources were collected and analyzed. Primary data for analysis were collected using the interview technique. Six patients with heart rhythm disorders were included in the research. Secondary data were obtained by reviewing domestic and foreign professional literature and online databases. Results: The interviewees find the disease to be a daily obstacle to their tasks and life activities. They arrived at the diagnosis through tests. They perceive holistic treatment as being treated in its entirety, together with their lifestyle. The interviewees face worsening of the disease and risks of additional life-threatening complications. The interviewees find preventive examinations and educational programs useful. Only one interviewee participates in educational programs. They visit the referral clinic regularly and are satisfied with the treatment. Discussion: It was found that the presence of a heart rhythm disorder represents constant adjustments to daily activities for patients. Treatment until the disease is confirmed was carried out with various diagnostic and therapeutic examinations, which were similar in all interviewees. The examinations are interconnected and, if necessary, are performed several times to obtain new findings. Patients perceive the comprehensive treatment of their disease as covering the whole patient, together with the disease, including their habits, physical and mental state, daily tasks and lifestyle. Complications that patients encounter on a daily basis are palpitations, dizziness, nausea, chest pain, heart palpitations, syncope. All of this can lead to sudden cardiac death or ischemic brain death. Patients find preventive examinations important, but they do not participate in additional education enough. Reference clinics regularly monitor and control the health status of patients, but they do not have a specially treated group for heart rhythm disorders.
