Title:Etične dileme medicinskih sester pri obravnavi pacientov v paliativni oskrbi : magistrsko delo študijskega programa druge bolonjske stopnje Zdravstvene vede – smer Zdravstvena nega

Abstract:Uvod: Magistrsko delo obravnava etične dileme, s katerimi se medicinske sestre srečujejo pri svojem delu v paliativni oskrbi. Paliativna oskrba je namenjena izboljšanju kakovosti življenja pacientov z neozdravljivimi boleznimi, zato se medicinske sestre pogosto znajdejo pred zahtevnimi etičnimi odločitvami. Metode: Izvedena je bila kvantitativna raziskava z vprašalnikom. Sodelovalo je 132 medicinskih sester, zbiranje podatkov je potekalo maja 2025. Glavni cilj je bil ugotoviti, kako v paliativni oskrbi etične dileme vplivajo na medicinske sestre. Rezultati: Raziskava je potrdila, da so etične dileme stalnica v praksi medicinskih sester. Najbolj pogosta etična dilema je vezana na željo pacienta po miru, tišini in samoti ob motečih obiskih, sledijo pa ji dileme, povezane z izvajanjem postopkov in posegov, ki niso vedno v skladu z željami in potrebami pacienta. Te ugotovitve poudarjajo, da se medicinske sestre pogosto znajdejo v položaju, ko morajo izbirati med dvema enako težkima, slabima ali nemogočima možnostma. Razprava: Za ugotovitve je značilno, da se etične dileme najpogosteje pojavljajo na stičišču pacientove avtonomije in strokovne dolžnosti. Ključna ugotovitev, ki jo potrjuje sprejeta četrta hipoteza, je statistično značilna soodvisnost med pogostostjo izpostavljenosti etičnim dilemam in njihovim negativnim vplivom na zdravje in počutje medicinskih sester. Na strokovnem nivoju bi bilo potrebno uvesti redne etične posvete in supervizije. Na regulativnem in sistemskem nivoju pa bi bilo treba oblikovati jasnejše protokole za reševanje etičnih konfliktov in izboljšati organizacijsko kulturo, saj ugotovitve kažejo, da so vzroki za moralno stisko sistemske narave in zahtevajo strukturne rešitve.

Abstract:Introduction: This master's thesis addresses the ethical dilemmas encountered by nurses in their work within palliative care. Palliative care aims to improve the quality of life for patients with incurable diseases; therefore, nurses frequently face demanding ethical decisions. Methods: A quantitative study was conducted using a questionnaire. 132 nurses participated, and data collection took place in May 2025. The main objective was to determine how ethical dilemmas in palliative care affect nurses. Results: The research confirmed that ethical dilemmas are a constant in nursing practice. The most common ethical dilemma relates to the patient's desire for peace, silence, and solitude in the face of intrusive visits, followed by dilemmas related to performing procedures and interventions that are not always in accordance with the wishes and needs of the patient. These findings highlight that nurses often find themselves in a position where they must choose between two equally difficult, poor, or impossible options. Discussion: The findings indicate that ethical dilemmas most frequently occur at the intersection of patient autonomy and professional duty. A key finding, confirmed by the accepted fourth hypothesis, is a statistically significant correlation between the frequency of exposure to ethical dilemmas and their negative impact on the health and well-being of nurses. At a professional level, it would be necessary to implement regular ethical consultations and supervisions. At the regulatory and systemic levels, clearer protocols for resolving ethical conflicts should be developed, and organizational culture should be improved, as the findings show that the causes of moral distress are systemic in nature and require structural solutions.
