Title:Učinki segmentalne pozicijske dihalne vadbe na gibljivost cervikalne in torakalne hrbtenice pri osebah s kronično bolečino v cervikalni hrbtenici : diplomsko delo visokošolskega strokovnega študijskega programa prve bolonjske stopnje Fizioterapija

Abstract:Kronična bolečina v cervikalni hrbtenici predstavlja enega najpogostejših vzrokov za motnje gibalnega sistema pri odraslih in pomembno prispeva k globalni zdravstveni obremenitvi. Standardni fizioterapevtski postopki vključujejo manualno terapijo in terapevtsko vadbo, katerih dolgoročni učinki ostajajo neraziskani. Segmentalna pozicijska dihalna vadba predstavlja inovativen terapevtski pristop, ki prek aktivacije diafragme, fascialne kontinuitete in nevrofizioloških mehanizmov freničnega živca deluje tudi na cervikalni segment hrbtenice. Namen diplomske naloge je bil ovrednotiti učinke osemdnevnega programa segmentalne pozicijske dihalne vadbe na gibljivost cervikalne in torakalne hrbtenice ter na intenzivnost bolečine pri osebah s kronično bolečino v cervikalni hrbtenici. V raziskavi je sodelovalo 23 preiskovancev, naključno razdeljenih v testno (n = 13) in kontrolno skupino (n = 10). Gibljivost je bila merjena z digitalnim goniometrom EasyAngle, intenzivnost bolečine pa z VAS lestvico. Rezultati so pokazali statistično značilno izboljšanje gibljivosti cervikalne hrbtenice pri rotaciji, lateralni fleksiji in fleksiji v testni skupini, medtem ko v kontrolni skupini statistično značilnih sprememb ni bilo. Pri torakalni hrbtenici se je statistično značilno izboljšala gibljivost v smeri fleksije in lateralne fleksije v desno. Intenzivnost bolečine se je v testni skupini zmanjšala za 2,00 enoti po VAS lestvici, kar presega minimalno klinično pomembno razliko. Ugotovitve podpirajo vključevanje segmentalne pozicijske dihalne vadbe v fizioterapevtsko obravnavo kronične bolečine v cervikalni hrbtenici.

Abstract:Chronic neck pain is one of the most common causes of musculuskeletal disorders in adults and contribues significantly to the global burden of diease. Standard physiotheraphy approaches include manual therapy an therapeutic exercise, however their long term effects remain insufficiently studied. Segmental positional breathing exercise represents an innovative therapeutic approach that acts on the cervical segment of the spine through diaphragm activation, fascial continuity and the neurophysiological mechanisms of the phrenic nerve. The purpuse of this thesis was to evaluate the effects of an 8-day segmental positional breathing exercise program on cervical and thoracic spine mobility and pain intensity in individuals with chronic cervical spine pain. The study included 23 participants divided into an experimental group (n = 13) and a control group (n = 10). Spinal mobility was measured using the EasyAngle digital goniometer and pain intensity was assessed using the Visual Analogue Scale (VAS). Results demonstrated statistically segnificant improvements in cervical spine mobility in rotation, lateral flexion and flexion in the experimental group, while no statistically significant changes were observed in the control group. In the thoracic spine, flexion and right lateral flexion improved significantly. Pain intensity in the experimental group decreased by 2,00 units on the VAS scale, exceeding the minimal clinically important difference. The findings support the inclusion of segmental positional breathing exercise in physiotherapeutic treatment of chronic cervical spine pain.
