Title:Fizioterapevtska obravnava bolnikov z multiplo sklerozo

Abstract:Multipla skleroza (MS) je kronična vnetna bolezen osrednjega živčevja, za katero je značilnih več potekov in stopenj. Bistvo obravnave bolnikov z MS, ki imajo težave s kronično šibkostjo, nestabilno hojo in slabšo koordinacijo, je fizioterapevtska obravnava. Le-ta med drugim vključuje kinezioterapijo, respiratorno fizioterapijo, protibolečinsko terapijo, hidroterapijo, raztezanje in zmanjševanje oviranosti pri hoji z uporabo pripomočkov za hojo. Metoda dela v nalogi je bila pregled literature. Angleška literatura je bila iskana v podatkovni bazi PubMed. Pregled ustrezne literature je pokazal, da je optimalen fizioterapevtski program za obravnavo bolnika z MS prilagojen težavam in ciljem posameznega bolnika. Skupinske vadbe pozitivno vplivajo na ravnotežje, funkcionalni status, spastičnost, utrujenost in kakovost življenja oseb z MS. FES (funkcionalna električna stimulacija) statistično pomembno izboljša padajoče stopalo in stabilnost kolka, zmanjša vpliv MS in pogostost padcev. S treningom prostovoljne maksimalne mišične kontrakcije lahko dolgotrajno povečamo mišično moč bolnikov z MS. S treningom inspiratornih in ekspiratornih mišic lahko bolniki z MS izboljšujejo mišično moč dihalnih mišic ter tako zmanjšujejo utrujenost. Terapevtska vadba v vodi povečuje funkcionalne sposobnosti, ravnotežje in zaznavanje utrujenosti pri bolnikih z MS. Iz uporabljene literature lahko sklepamo, da ima fizioterapevtska obravnava bolnikov z MS velik pomen za kakovost njihovega življenja. Na področju optimalnih metod in postopkov pri fizioterapevtski obravnavi bolnikov z MS so potrebne nadaljnje raziskave.

Title:Physiotherapeutical treatment of patients with Multiple Sclerosis
Abstract:Multiple sclerosis (MS) is a chronic demyelinating inflammatory illness of the central nervous system characterized by several courses and stages. The essence of the treatment of patients with MS who has problems with chronic weakness, unstable walking, poor coordination and proprioception is physiotherapeutic treatment. Physiotherapy for patients with MS includes respiratory physiotherapy, pain therapy, hydrotherapy, stretching and the reduction of gait instability with use of walking accessory should be included. The working method is literature review. English literature has been searched using the PubMed, database. The optimal physiotherapy program for treating a patient with MS is individualized. Group exercises have a positive effect on balance, functional status, spasticity, fatigue and quality of life of people with MS. FES (functional electrical stimulation) statistically significantly improves flat sole and hip stability, reduces the effect of MS and the incidence of falls. With the training of inspiratory and expiratory muscles, MS patients can improve the muscular strength of the respiratory muscles and also reduce fatigue. Therapeutic exercise in water increases functional ability, balance, and fatigue detection in patients with MS. With maximal voluntary muscle contraction the muscular strength of patients with MS can be increased. From literature we can make conclusion, that the integral treatment of patients with MS is crucial. Further research is needed about pharmacological therapies and physiotherapy with patients with MS.
