Abstract:Vključitev predoperativne rehabilitacije, ki vsebuje predoperativno vadbo in edukacijo pacienta, ima velik pomen v rehabilitaciji. Ob nadzoru fizioterapevta pacient pridobi potrebne informacije, ki mu zelo pomagajo skozi celoten potek do operativnega posega in med nadaljnjo rehabilitacijo. Raziskave kažejo, da primerna edukacija pacienta zmanjšuje občutek anksioznosti in pojav nerealnih pričakovanj, ki lahko zavedejo pacienta, kar lahko privede do padca volje in motivacije med rehabilitacijskim procesom. Predoperativna vadba poskrbi za boljšo psihofizično kondicijo pacienta pred operativnim posegom, pozitivno vpliva na celoten mišično-skeletni sistem in zagotovi boljše predispozicije za pričetek zgodnje rehabilitacije. Zelo pomembna je tudi takojšna postoperativna rehabilitacija. Proces rehabilitacije kolka po vstavitvi TEP je dolgotrajen postopek, ki lahko traja tudi več kot leto dni. Funkcija kolka se izboljša, vendar v večini primerov ne doseže optimalnih ali normativnih vrednosti, predvsem gibljivost kolka ostane rahlo omejena. Kljub temu večina pacientov doseže mnogo boljše rezultate kot pred operativnim posegom, postanejo bolj samostojni, imajo manj bolečin, kar se odraža v dvigu kakovosti njihovega življenja. Zaradi konstantnega naraščanja števila operativnih posegov z vstavitvijo TEP kolka je potrebno, da z različnimi tehnikami vzdržujemo nivo kakovosti rehabilitacije. To najlažje dosežemo z individualnim pristopom, nenehnim izobraževanjem fizioterapevtov in ostalih zdravstvenih delavcev ter multidisciplinarnim pristopom različnih zdravstvenih delavcev in ustanov.

Title:An evidence-based physiotherapy approach to treating patients with total hip replacement
Abstract:Preoperative rehabilitation encompassing exercises as well as patient education is of significant importance. Under the tutelage of a physiotherapist, a patient obtains all the necessary information and guidelines, which eases the patient and helps them with their preparation before surgery as well as with their postoperative rehabilitation. Research suggests that primary patient education helps lower patient anxiety levels as well as lower their often surreal expectations which can quickly lead to a lack of motivation and sap their will during their rehabilitation process. Preoperative exercise improves the patient’s psychophysical state and has an all-around positive impact on their entire musculoskeletal system, thus guaranteeing a better basis for the patient’s rehabilitation and recovery following the surgical procedure. Rehabilitation following total hip replacement (total hip endoprothesis or TEP) is a long-term process, which can take up to more than a year. A patient’s hip becomes more functional, however, in most cases, it does not reach optimal or normal values mostly due to hip range of motion (ROM), which commonly stays slightly limited. Despite this, the majority of patients report very good results versus their preoperative condition, they become more independent and, subsequently, their quality of life (QOL) improves. The constant rise in the number of TEP hip replacement procedures gives rise to guarantee and maintain a high quality level of postoperative rehabilitation. The best way to achieve this is with an individually-tailored approach, constant learning and training of physical therapists and other healthcare workers, as well as by working in a multidisciplinary team of various healthcare workers including several healthcare institutions.
