Title:Vrednotenje kinestezije vratu pri bolnikih z bolečinami v vratu in vrtoglavicami

Abstract:Uvod in namen: Cervikogene vrtoglavice so pogost simptom, ki spremljajo posameznike z bolečinami v vratu. Izsledki raziskav nakazujejo, da bi lahko pojav cervikogenih vrtoglavic pripisali neskladju senzoričnega dotoka iz vratnih proprioceptorjev, vestibularnega aparata in vidnega sistema. Namen diplomske naloge je bil prikazati razlike v kinestetičnem zaznavanju vratne hrbtenice med zdravimi posamezniki ter bolniki z bolečinami v vratu z in brez vrtoglavic. Metode: V študiji je sodelovalo 71 preiskovancev, od tega 25 zdravih in 46 s kronično bolečino v vratu (21 s pridruženo vrtoglavico). Kinestezija vratu je bila vrednotena s pomočjo Metuljnega testa in spremenljivkami prehitevanja, zaostajanja, časa na referenci in gladkosti gibanja. Rezultati: Analiza varianc je pokazala statistično značilne razlike za vse opazovane spremenljivke. Post-hoc analize so pokazale statistično značilne razlike med skupino zdravih preiskovancev in obema skupinama bolnikov za večino spremljanih spremenljivk. Statistično značilne spremenljivke med obema skupinama bolnikov so bile prisotne za spremenljivke zaostajanja, prehitevanja in amplitudne napake. Uporabnost: Rezultati raziskave pomembno prispevajo k razumevanju odnosa med kinestetičnimi deficiti in težavami z vratno hrbtenico ter spremljajočimi simptomi. Posledično je smiselno v rehabilitacijske protokole vključiti vaje za izboljšanje kinestetičnega zaznavanja. Omejitve: Uporaba slikovne diagnostike bi lahko doprinesla k boljšemu razumevanju strukturnih sprememb in njihovih povezav s funkcionalnimi testi in simptomi, povezanimi s cervikogeno vrtoglavico.

Title:Assessment of kinaesthetic awareness in neck pain patients with and without dizziness
Abstract:Introduction and purpose: Cervicogenic dizziness is commonly reported in neck pain patients. Basic studies suggest that these could be due to sensory mismatch caused by discrepancies between different proprioceptors located in various cervical structures, vestibular and visual system. The purpose of this research was to compare different aspects of kinaesthetic awareness between healthy subjects and neck pain patients with and without dizziness. Methods: Seventy-one subjects were enrolled in the study, of which 25 were healthy and 46 with chronic neck pain (21 with dizziness). The Butterfly test was used to assess kinaesthetic awareness during head and neck movements in an unpredictable manner. Parameters of Time-on-target, Overshoot, Undershoot and Smoothness of movement were used to compare between the groups. Results: Analysis of variance showed statistically significant differences between all groups for all parameters observed. Post-hoc tests revealed differences between healthy subjects and both patient groups in majority of parameters observed. Differences between two patient groups were observed for undershoot, overshoot and amplitude accuracy parameters of the Butterfly test. Usability: Results importantly upgrade current knowledge about the relationship between kinaesthetic deficits and cervical spine related symptoms. Therapeutic protocols should target different aspects of kinaesthesia. Limitations: Different imaging techniques could provide better understanding of structural impairments and their relationship with functional tests and cervical spine related symptoms.
