Title:NOVOSTI PRI REHABILITACIJI PO VSTAVITVI TOTALNE ENDOPROTEZE KOLENA

Abstract:V diplomskem delu smo raziskovali klinično pot zdravljenja po vstavitvi totalne endoproteze kolena ter ugotavljali moderne smernice in trende v rehabilitaciji. Cilj diplomskega dela je bil ugotoviti, ali kirurgija TEP s pospešenim okrevanjem da primerljive ali boljše rezultate kot klasičen potek zdravljenja. V ta namen smo si zastavili raziskovalno vprašanje, ali koncept kirurgije s pospešenim okrevanjem prispeva k izboljšanju rehabilitacije, in potrdili hipotezo, da s predoperativno pripravo in zgodnjo mobilizacijo pacienta vplivamo na boljši potek rehabilitacije. Raziskavo smo v kvalitativnem delu opisali z domačimi in tujimi viri. Na Ortopedski kliniki v Ljubljani smo spremljali pacienta v vseh fazah rehabilitacije in poročali o končnih ugotovitvah. Opisali smo klinični potek zdravljenja pacienta, ki smo ga po operaciji TEP kolena zdravili po klasični metodi, in pacienta, ki smo ga zdravili po konceptu kirurgije s pospešenim okrevanjem. KSPO omogoča zmanjševanje predoperativnih in pooperativnih zapletov ter stopnje bolečine in skrajševanje ležalne dobe po operativnem posegu. Možnosti optimizacije pa so se pokazale na različnih področjih predoperativne in pooperativne obravnave pacienta (Larsen, 2008). Glede na pridobljene podatke iz literature in ob spremljanju kliničnega primera smo ugotovili, da KSPO omogoča hitrejšo rehabilitacijo in daje primerljive ali celo boljše končne rezultate zdravljenja. To omogoča usklajeno sodelovanje večdisciplinarne ekipe in je rezultat izboljšav v operativnih tehnikah protibolečinske terapije, usmerjene fizioterapije in boljše informiranosti pacienta. Če je pacient dobro informiran o poteku rehabilitacije in je predvsem z vidika fizioterapevtske obravnave predhodno primerno pripravljen v smislu naučenih vaj in čim boljše fizične pripravljenosti, lahko pričakujemo hitrejše okrevanje in povečano zadovoljstvo pacienta samega z rezultatom rehabilitacije.

Title:NOVELTIES IN REHABILITATION AFTER TOTAL KNEE ARTHROPLASTY
Abstract:The graduation thesis investigates the clinical path of treatment after the insertion of a total knee endoprosthesis and identifies modern guidelines and trends in rehabilitation. The aim of the thesis was to determine whether enhanced recovery after TEP surgery gives comparable or better results than the classic course of treatment. To this end, the research question was set as to whether the concept of enhanced recovery after surgery (ERAS) contributes to improved rehabilitation, and the hypothesis was confirmed that preoperative preparation and early patient mobilisation influence a better course of rehabilitation. The qualitative part of the research was described with domestic and foreign sources. A patient of the Orthopaedic Clinic in Ljubljana was monitored in all phases of rehabilitation and a report was written on the final findings. The clinical course of treatment of the patient who was treated according to the classical method after TEP knee surgery, and a patient who was treated according to the concept of ERAS were described. The ERAS protocol enables the reduction of preoperative and postoperative complications, as well as the level of pain and shortening of the lying period after surgery. Possibilities for optimisation, however, have been demonstrated in various areas of preoperative and postoperative patient treatment (Larsen, 2008). Based on the data obtained from the literature and monitoring the clinical case, it was found that the ERAS protocol allows faster rehabilitation and gives comparable or even better end results of treatment. This allows for the coordinated collaboration of a multidisciplinary team and is the result of improvements in operative techniques of pain management, targeted physiotherapy, and better patient information. If the patient is well informed about the course of rehabilitation and is previously prepared in terms of learned exercises and the best possible physical fitness, it is possible to expect faster recovery and increased satisfaction of the patient with the result of rehabilitation.
