Title:Model varnostne kulture na področju varnosti pacientov : magistrska naloga

Abstract:Raziskovalno vprašanje (RV): V današnjem času se v zdravstveni dejavnosti čedalje več pozornosti namenja zmanjšanju varnostnih incidentov med zdravstveno obravnavo, saj ti predstavljajo veliko tveganje za pac iente. Pri tem ima veliko vlogo varnostna kultura v zdravstveni ustanovi. V magistrski nalogi poskušamo odgovoriti na raziskovalno vprašanje, kateri dejavniki vplivajo na varno vedenje zaposlenih zdravstvenih delavcev v zdravstveni ustanovi. Namen: Namen in cilji raziskave so prepoznati dejavnike vpliva na varno vedenje zdravstvenih delavcev v zdravstveni ustanovi in jih povezati v model varnostne kulture na področju varnosti pacientov, ki bo vodstvu zdravstvene ustanove v pomoč pri izboljševanju varnostne kulture v njihovi organizaciji. V raziskavo smo vključili zdravstveno osebje ene izmed specializiranih bolnišnic v Sloveniji. Metoda: V raziskavi je sodelovalo 165 zdravstvenih delavcev in sodelavcev. Za testiranje hipotez smo uporabili standardizirani an ketni vprašalnik organizacije AHRQ (Agency for Healthcare Research and Quality), ki meri varnostno kulturo v bolnišnicah. Podatke smo obdelali s pomočjo različnih statističnih metod. Uporabili smo metode deskriptivne in inferenčne statistike. Rezultati: Re zultati raziskave so pokazali, da na varno vedenje zdravstvenega osebja vpliva jo njihova osebna pripravljenost na varno vedenje, subjektivne norme, ki se izražajo kot pričakovanja vodstva , ter samonadzor posameznika nad lastnim vedenjem, pri čemer je pomem bno, da delovno okolje zagotavlja sredstva in priložnosti za varno vedenje ter odpravlja ovire za takšno vedenje. Na osnovi pridobljenih rezultatov smo definirali model varnostne kulture na področju varnosti pacientov v zdravstveni ustanovi. Organizacija: Model varnostne kulture na področju varnosti pacientov bo vodstvu zdravstvene ustanove v pomoč pri oblikovanju programa varnostne kulture, saj zajema bistvene elemente vpliva na prepričanja zdravstvenega osebja glede varnosti v zdravstveni ustanovi, ki so temelj varnega vedenja. Model je za vodstvo uporaben povsod, kjer želijo vplivati na vedenje zdravstvenega osebja, zato je koristen tudi pri načrtovanju in uvajanju izboljšav na katerem koli področju, ne le na področju varnosti pacientov. Družba: Menimo, d a je model varnostne kulture na področju varnosti pacientov uporaben v prav vseh zdravstvenih ustanovah, saj se povsod soočajo s tveganjem nastanka varnostnih incidentov. Z uporabo modela bo vsaka zdravstvena ustanova lahko izboljšala varno vedenje zdravst venega osebja, zmanjšala število varnostnih incidentov in povečala varnost za paciente ter tako dvignila kakovost zdravstvenih storitev. Originalnost: V zdravstveni dejavnosti enake ali podobne raziskave o varnem vedenju zdravstvenih delavcev v okviru varn ostne kulture na področju varnosti pacientov med domačo in tujo literaturo nismo zasledili. Omejitve/nadaljnje raziskovanje: Raziskava je bila izvedena le na področju ene zdravstvene ustanove, na vzorcu 165 zaposlenih zdravstvenih delavcev. Zanimivo bi bil o v enako raziskavo vključiti več slovenskih zdravstvenih ustanov. V prihodnosti bi bilo dobro raziskati vpliv na varno vedenje med posameznimi profili v zdravstvu, saj predpostavljamo, da med njimi obstajajo razlike.

Abstract:Research question (RQ): Nowadays, the field of healthcare pays increasingly more attention to reducing safe ty incidents during medical treatment, as these present a major risk to patients. In doing so, a major role is played by the safety culture in a health care institution. In the Master's thesis, we try to answer the research question of which factors affect safe behaviour of the employees in a health care institution. Purpose: The aim and objectives of this research were to identify the factors that affect safe behaviour of the employees in a health care institution and connect them into a safety culture mo del in the field of patient safety, which will assist the health care institution's management in improving the safety culture within the organisation. The research included the health care staff working in one of the specialist hospitals in Slovenia. Meth od: A total of 165 health care workers and co - workers participated in the research. To test the hypotheses, we used the standard questionnaire developed by the AHRQ (Agency for Healthcare Research and Quality) which measures safety culture in hospitals. Th e data was processed using different statistical methods. We used the methods of descriptive and inferential statistics. Results: The research results showed that safe behaviour of the health care staff is affected by their personal willingness to behave s afely, subjective norms expressed in the form of expectations from the management, and self - control of each individual over his/her own behaviour, and it is important that the working environment provides the means and opportunities for safe behaviour, red ucing the obstacles for such behaviour. Based on the obtained results, we defined the safety culture model in the field of patient safety in a health care institution. Organisation: The safety culture model in the field of patient safety will assist the he alth care institution's management in designing a safety culture programme, as it comprises the main elements affecting the beliefs of the health care staff regarding safety in a health care institution, which are fundamental to safe behaviour. The managem ent can use the model in all situations where they want to affect the behaviour of the health care staff. Therefore, the model is useful also in the planning and introduction of improvements in any field, not only in the field of patient safety. Society: W e believe that the safety culture model in the field of patient safety can be used in all health care institutions, since all of these face the risk of the occurrence of safety incidents. By using the model, every health care institution can improve safe b ehaviour of its health care staff, reduce the number of safety incidents and increase patient safety, thus improving the quality of health care services. Authenticity: Reviewing the local and foreign literature, we have not found any identical or similar r esearch on the safe behaviour of the health care staff in a health care institution in the context of safety culture in the field of patient safety. Limitations/further research: The research was conducted only in one health care institution on a sample of 165 employed health care workers. It would be interesting to include more Slovenian health care institutions in the same research. In the future, it would be wise to research the effect on the safe behaviour among individual profiles in healthcare, as we presume there are differences between them.
