Title:Znanje in stališča medicinskih sester o paliativni oskrbi : diplomsko delo visokošolskega strokovnega programa prve stopnje "zdravstvena nega"

Abstract:Teoretična izhodišča: Paliativna oskrba temelji na obveščenosti pacienta o njegovi bolezni, vključenosti pacienta v odločanje o zdravstveni obravnavi in podpori svojcem. Z različnimi oblikami pomoči paliativna oskrba omogoča pacientu dostojno življenje do smrti. Medicinska sestra mora poznati in razumeti proces dojemanja in soočanja pacienta z napredovalo boleznijo, osnovno izhodišče paliativne oskrbe je interdisciplinaren timski pristop. Cilj: Glavni cilj diplomskega dela je ugotoviti, ali medicinske sestre poznajo paliativno oskrbo na način, kot je opredeljena v Državnem programu paliativne oskrbe, njihovo vključenost v izvajanje paliativne oskrbe in samooceno znanja medicinskih sester za izvajanje paliativne oskrbe. Ocenjevali smo predstave medicinskih sester glede znanja o paliativni oskrbi. Metoda: Raziskava je temeljila na deskriptivni metodi kvantitativnega raziskovanja. Uporabili smo metodo anketiranja. Vprašalnik je bil zaprtega tipa, vseboval je 32 trditev, do katerih so se anketiranci opredeljevali na lestvici od 1 do 5. Med diplomirane medicinske sestre in tehnike zdravstvene nege smo po metodi namenskega vzorca razdelili 150 anketnih vprašalnikov, vrnjenih je bilo 76, kar predstavlja 50,6 % realizacijo vzorca. Vključenih je bilo 72 žensk in 4 moški. Rezultati: Anketiranci so lastno znanje o paliativni oskrbi ocenili kot srednje dobro (PV=3,58), le 8 % anketirancev se je popolnoma strinjalo, da imajo dovolj znanja na področju paliativne oskrbe. V zadnjih petih letih so se malo izobraževali iz paliativne oskrbe (PV=2,41), 40 % anketirancev se je popolnoma strinjalo, da je za medicinske sestre zelo pomembno podiplomsko izobraževanje na tem področju. Anketiranci so v 56 % bolečino opredelili kot enega izmed najpomembnejših simptomov pri pacientih v paliativni oskrbi. Da huda telesna bolezen, kot je npr. rak, globoko poseže v življenje obolelih in njihovih bližnjih, se je popolnoma strinjalo 67 % anketirancev. Rezultati raziskave kažejo, da ni statistično značilnih razlik v odnosu in stališčih do paliativne oskrbe med medicinskimi sestrami s srednješolsko in visokošolsko izobrazbo. Razprava: Z izvedeno raziskavo smo ugotovili, da se anketiranci v raziskavi malo izobražujejo na področju paliativne oskrbe in zdravstvene nege. Anketiranci ocenjujejo, da nimajo dovolj znanj na področju paliativne zdravstvene nege. To je zlasti zaskrbljujoče, ker mnogi izmed vključenih v raziskavi delajo s pacienti, ki imajo neozdravljive kronične bolezni. Ugotovili smo, da se medicinske sestre vključujejo v izvajanje paliativne oskrbe in zdravstvene nege vsakodnevno, saj skrbijo za te paciente. Medicinske sestre se zelo zavedajo, kako pomembno je lajšanje bolečine ter drugih spremljajočih simptomov, ki jih imajo pacienti ob napredovali neozdravljivi bolezni. Raziskava daje usmeritve vodstvom zdravstvene nege, da v prihodnosti načrtujejo izobraževanja na tem področju.

Abstract:Theoretical basis: Palliative care is based on the information of the patient about his disease, including the patient in the decisions concerning his medical treatment and supporting the relatives. With different kinds of help palliative care enables the patient to live decent life till his death. The nurse has to know and understand the process of patient's perception and facing with his progressed disease. The team approach is basic for palliative care. The goal: The mail goal of my diploma work is to discover if the nurses know the palliative care as it is defined in State programme of palliative care. Their inclusion in the implementation of palliative care and the self-evaluation of their knowledge concerning the palliative care are also discussed. Method: The research was based on the descriptive method of quantitative researching. The method of questionnaire was used. The questionnaire was closed type and it contained 32 statements andthe respondents have to define them with the range from 1 do 5. The questionnaire was given to graduated nurses and the medical technician and by the method of dedicated samples we give them 150 questionnaire. We got back 76, this is 50,6 % of the sample. 72 women and 4 men were included. Results: The respondents accessed their knowledge about palliative care as medium good (PV = 3,58), only 8 % of respondents thought they have enough knowledge about palliative care. In last five years there was only a little amount of education concerning the palliative care. 40 % of the respondents absolutely agreed that postgraduate education concerning palliative care is important for the nurses. 56 % of the respondents defined the pain as the most important symptom by patients in palliative care. 67 % agreed that the physical disease (cancer) deeply intervenes in the entire life of the patient and his relatives. The results shows that there is no obvious difference in the relation and the position towards the palliative care between the graduated nurses and those with secondary school. Discussion: The research shows that the respondents do not educate a lot about palliative care and nursing care. They evaluate that they do not have enough knowledge about palliative care. This is concerning becausemany of respondents work with the patients that have incurable chronic diseases. The nurses are the part of the palliative care and nursing care every day and they are very well aware how important is the relief of the painand other symptoms that the patients have while the incurable disease progress. The research can give the orientation to the authorities in nursingcare to increase the amount of education on this field.
