Title:Primerjava operativnega in konservativnega zdravljenja pacientov s cervikalno radikulopatijo

Abstract:Uvod in namen: Najpogostejši vzrok cervikalne radikulopatije (CR) je okvara vratne živčne korenine pri izstopu iz hrbtenjače. Namen diplomske naloge je analiza pacientov s CR in primerjava dolgoročne uspešnosti operativnega in konservativnega zdravljenja. Metode: Retrospektivna študija s pregledom medicinske dokumentacije in opisom demografskih, anamnestičnih in kliničnih značilnosti pacientov z elektrofiziološko potrjeno okvaro vratnih živčnih korenin ter primerjava dolgoročne uspešnosti operativnega in konservativnega zdravljenja. Rezultati: Najpogosteje gre za okvaro sedme vratne živčne korenine (C7) neodvisno od dominantnosti zgornje okončine, pogosteje pa so prizadeti moški po 50. letu starosti. Najpogostejši simptom je bolečina v vratu in roki z mravljinčenjem v zgornjem udu. Uspešnost operativnega zdravljenja in uspešnost fizioterapije je primerljiva. Obe metodi značilno zmanjšata bolečino v vratu in roki ter pogostost pojavljanja težav, izboljšata stanje in izpolnita pričakovanja pacientov. Za paciente brez zdravljenja je značilnejša manjša začetna bolečina, ki se pri večini ne spremeni in je stalna. Takšen pristop praviloma ne izpolni pričakovanj pacientov. Uporabnost: Rezultati so uporabni v vsakodnevni klinični praksi. Omejitve: Retrospektivna raziskava.

Title:Surgical versus conservative treatment in patients with cervical radiculopathy
Abstract:Introduction and purpose: Cervical radiculopathy (CR) is most commonly caused by the compression of spinal nerve root in the region of neural foramen. The purpose of this study is to analyse patients with CR and compare long-term effectiveness of surgical versus conservative treatment. Methods: Retrospective analysis of demographic, anamnestic and clinical characteristics of patients with electro-physiologically confirmed CR and comparison of long-term effects between conservative and surgical treatment. Results: The 7th cervical nerve root (C7) is most commonly affected, independently of patients’ hand dominance. The majority of patients are male and over the age of 50. The most common symptoms are neck and arm pain with upper limb numbness. Surgical and conservative treatments are comparatively successful. Both methods significantly decrease neck and upper limb pain, reduce and improve reoccurring symptoms and fulfil patients’ expectations. Patients with no treatment experience lower pain intensity at the beginning which does not improve and is present in their everyday life. This approach does not meet patients’ expectations. Usability: Results are applicable in everyday clinical practice. Limitations: Retrospective study design.
