Title:Preventivni in kurativni učinki kognitivno-vedenjskega svetovanja v primeru kronične bolezni : magistrska naloga

Abstract:Pojavnost kroničnih bolezni in rakavih obolenj se močno dviguje. Posameznik je namreč danes izpostavljen vedno večji količini stresa. Vendar pa se le redki zavedajo vpliva negativnih avtomatskih misli na vedenja in čustvovanja. ki posledično ogrožajo njihovo duševno in telesno zdravje. Magistrska naloga raziskuje, kakšni so preventivni in kurativni učinki kognitivno-vedenjskega svetovanja (v nadaljevanju KVS) ob nastopu kronične bolezni. V teoretičnem delu opisujemo psihološke mehanizme nastanka bolezni, teorijo KVS in terapijo ter izhodišča svetovanja ob nastopu bolezni. V empiričnem delu pa z analizo sekundarnih podatkov raziskujemo KVS pri soočanju z diagnozo, ki ogroža življenje bolnika (rak, kronična bolezen) – kakšen vpliv ima na pojavnost bolezni stres kot posledica neprilagojene kognitivne strukture, ali je lahko svetovanje dobra komplementarna oblika zdravljenja ter kako lahko izboljša kakovost življenja bolnika, ki se spopada z motnjami razpoloženja in utrujenostjo. Analiza rezultatov potrjuje domnevo, da neprilagojena kognitivna struktura lahko povzroči nastanek bolezni ali pa jo celo pospeši. Prav tako se je izkazalo, da ima KVS komplementarno vlogo pri zdravljenju kroničnih bolezni, vendar z določenimi omejitvami (učinkoviteje ob pričetku terapije nekaj mesecev po nastopu bolezni kot neposredno ob diagnozi, pomembnost trajanja). Kot zadnje pa smo potrdili, da je področje utrujenosti in razpoloženjskih težav tisto, kjer lahko KVS najbolj izboljša kakovost življenja bolnikov. Izkazalo se je, da psihoedukacija in učenje tehnik za prepoznavanje misli ter prepričanj pripomorejo k boljšemu spopadanju obolelega s stresom in depresijo, kar zmanjša vnetni odziv telesa ter pozitivno vpliva na razpoloženje in zmanjša utrujenost. Tako se bolniki lahko boljše soočajo z bremenom kronične bolezni po diagnozi. Zaključujemo, da ima KVS preventivne in kurativne učinke v primeru kronične bolezni.

Abstract:The incidence of chronic diseases and cancers is rising sharply. An individual today is exposed to an increasing amount of stress. However, few are aware of the impact of negative automatic thoughts that endanger their mental and physical health. Preventive and curative effects of cognitive-behaviour counselling (CBC) in case of chronic disease are therefore explored in this master's thesis. Psychological mechanisms of disease development, theory of CBC and key points of counselling at the onset of the disease are presented in the theoretical part. The role of CBC in dealing with life-threatening diagnose (cancer, chronic disease) was examined using the secondary data analysis in the empirical part. Furthermore, impact of stress as a result of a maladaptive cognitive structure on the incidence of the disease, whether counselling can be a good complementary form of treatment and how it can improve quality of life of a patient struggling with mood disorders and fatigue were also explored. Assumption that maladaptive cognitive structure can lead to development of disease or even accelerate it was confirmed in the analysis of results. It has also been shown that CBC can play a complementary role in treatment of chronic diseases, but with certain limitations (less effective if started too soon after diagnosis, importance of duration). Lastly, it was confirmed that fatigue and mood problems are areas where CBC can best improve patient quality of life. Psychoeducation, learning techniques to recognize negative automatic thoughts and beliefs, has been shown to help patients cope better with stress and depression, reducing the body's inflammatory response while also having a positive effect on mood and reducing fatigue. Thus, patients can better cope with the burden of chronic disease after diagnosis. To conclude, CBC has been shown to have preventive and curative effects in case of the occurrence of chronic disease.
