Title:Vpliv fizioterapevtske obravnave na celjenje rane ob epiziotomiji : diplomska naloga

Abstract:Porod je naravna in normalna izkušnja. Vaginalni porod spremljajo sočasna tveganja, saj se močno obremenijo mišice medeničnega dna (MMD) in presredek. Vsekakor pusti za seboj določeno posledice, še posebej takrat, kadar pride do rezanja presredka oziroma epiziotomije. Epiziotomija je kirurško povečanje vaginalne odprtine z rezom v drugem delu druge porodne dobe. Situaciji, pri katerih je rez nujno potrebno opraviti, sta operativno dokončanje vaginalnega poroda in pri medenični vstavi. Tudi kadar oseba, ki vodi porod (babica/porodničar) oceni, da je tkivo presredka trdo in neelastično, se lahko odloči za epiziotomijo. Znanih je sedem vrst epiziotomij. V Evropi je najpogosteje uporabljena mediolateralna epiziotomija. Bolečina in nelagodje ob celjenju rane zmanjšuje kakovost in funkcionalnost žensk. Pri pregledu literature smo ugotovili, da se je fizioterapevstka obravnava izkazala kot učinkovit pristop pri lajšanju težav ob epiziotomiji. Vaje za krepitev MMD so najučinkovitejša metoda za hitrejše celjenje epiziotomijske rane. Za zmanjšanje bolečin v presredku se najpogosteje uporablja fizikalna terapija. Terapevtski ultrazvok, transkutana električna nevro stimulacija in nizkoenergijski laser dajejo spodbudne rezultate pri lajšanju perinealne bolečine. Obravnava je v poporodnem obdobju vedno individualna in prilagojena posameznici. Le tako lahko dosežemo zastavljene cilje in ji zagotovimo kakovostno oskrbo ter čim hitrejšo vzpostavitev njenega zadovoljstva in zdravja.

Title:The effect of physiotherapy treatment on wound healing after episiotomy
Abstract:Childbirth is a natural and normal experience. Vaginal delivery is accompanied by risks, as it greatly burdens the pelvic floor muscles (PFM) and the perineum. It certainly leaves consequences, especially when it involves making a surgical incision into the perineum or an episiotomy. Episiotomy is the surgical enlargement of the vaginal orifice by making an incision during the last part of the second stage of labour. Two situations that require making an incision are an operative vaginal delivery and a breech presentation. When the person overseeing the delivery (midwife, obstetrician) assesses that the tissue of the perineum is rigid and stiff, they may decide to perform an episiotomy. There are seven types of episiotomies. The most commonly used type in Europe is mediolateral episiotomy. The pain and discomfort experienced by women as the wound heals reduce their functionality and quality of life. The literature review shows that physiotherapy has proved to be an effective approach to alleviating episiotomy-related problems. Exercises for strengthening PFM are the most effective method for speeding up the healing of an episiotomy wound. Physical therapy is most commonly used to alleviate perineal pain. Therapeutic ultrasound, transcutaneous electrical nerve stimulation and low-level laser therapy have yielded encouraging results in alleviating perineal pain. Postpartum care is always one-on-one and adapted to the individual. That is the only way to achieve the set goals, provide quality care, and restore the woman's satisfaction and health as quickly as possible.
