Abstract:
Teoretična izhodišča: Razvoj otroka se začne že v maternici, zato so materino duševno zdravje, stres in stresni dejavniki, psihosocialno okolje v nosečnosti in po rojstvu otroka ključnega pomena za zdravje otroka. Namen je raziskati vpliv prenatalnega in postnatalnega obdobja na pojavnost psihosomatske kožne bolezni atopijski dermatitis v zgodnjem otroštvu. Metoda: V diplomski nalogi smo uporabili kvalitativni pristop in v njem metodo analize sekundarnega gradiva. Gradivo smo pridobivali pomočjo podatkovnih baz PubMed, Google sholar, Google učenjak, DermMed. Uporabili smo tudi 21 strokovnih knjig. Iskanje literature je potekalo od meseca marca 2022 do julija 2023. Pri pregledu literature sta bila uporabljena prizma diagram in hierarhična ocena kakovosti literature. Rezultati: V končno analizo je bilo po 404.145 zadetkih vključenih 34 člankov in strokovnih besedil ter 21 strokovnih knjig, ki so ustrezali vključitvenim merilom. Hierarhično so najvišje na lestvici sistematični pregled korelacijskih/opazovalnih študij (6), posamezne korelacijske/ opazovalne študije (2), sistematični pregled opisnih kvalitativnih študij (6), posamične opisne kvalitativne študije (8), in mnenja avtorjev/ekspertnih komisij (12). Ugotovitve: V večini primerov članki in znanstvena dela govorijo o povezavi z okoljskimi, s psihosocialnimi in z genetskimi dejavniki, ki se iz matere prenašajo na še nerojenega otroka in pomembno vplivajo tudi v času zgodnjega razvoja otroka. V raziskavi smo na vsako opredeljeno temo predstavili znanstvene članke in literaturo o vplivu dejavnikov na pojavnost kožne bolezni pri otroku. Ugotovili smo tudi, da je tema izjemno široka in neraziskana predvsem na novih znanstvenih področjih, kot sta epigenetika in prenos travme na otroka. Predvsem študije govorijo o zdravstveni komponenti, zelo malo pa o čustvenih dejavnikih, ki sprožijo bolezen. Se pa raziskovalci strinjajo, da je atopijski dermatitis izjemno izčrpavajoča bolezen in pomembno vpliva na kakovost celotne družine, prav tako je velik finančni zalogaj. Psihološka podpora otroku in staršem ter vključitev obojih v izobraževalne programe lahko izboljšata kakovost življenja, stanje kože in srbež pri otroku.

THE INFLUENCE OF THE PRENATAL AND POSTNATAL PERIOD ON THE INCIDENCE OF ATOPIC DERMATITIS IN EARLY CHILDHOOD
Abstract:
Theoretical background: Child development begins while still in the womb, so the mother's mental health, stress, stressors, and the psychosocial environment during pregnancy and after birth are crucial for the child's health. This study aims to investigate the influence of the prenatal and postnatal periods on the incidence of the psychosomatic skin disease atopic dermatitis in early childhood. Method: In the thesis, we used a qualitative approach, namely secondary material analysis. The material was obtained from PubMed, Google Scholar, and DermMed databases. We also used 21 expert books. The literature search was conducted between March 2022 and July 2023. A prism diagram and hierarchical literature quality assessment were used in the literature review. Results: After 404,145 hits, the final analysis included 34 expert articles and texts, and 21 expert books that met the inclusion criteria. The highest ranked were systematic review of correlational/observational studies (6), individual correlational/observational studies (2), systematic review of descriptive qualitative studies (6), individual descriptive qualitative studies (8), and author/expert panel reviews (12). Observations: In most cases, articles and scientific papers discuss the connection with environmental, psychosocial, and genetic factors transmitted from the mother to the unborn child. They also have a significant impact during the child's early development. In this study, we have presented for each identified theme scientific articles and literature on the influence of factors on the incidence of skin disease in the child. We also found that the topic is extensive and unexplored, especially in new scientific areas such as epigenetics and transmitting trauma to the child. Studies mainly focus on the medical component but very little on the emotional factors that trigger the disease. However, researchers agree that atopic dermatitis is a highly debilitating disease that significantly impacts the entire family's quality of life and is a major financial burden. Psychological support for the child and the parents and including both in educational programs can improve the child's quality of life, skin condition, and itchiness.
